Job summary

UnitedHealth Group is seeking a Hospice RN Case Manager in Cottonwood, AZ to take full nursing responsibility for the Plan of Care, collaborating with physicians, social workers and other staff to ensure compassionate, patient- and family-centered end-of-life care.

The role requires an active RN license, CPR certification, and at least 1 year of clinical experience; a valid driver's license and reliable transportation are needed for in-field visits.

Qualifications

• RN licensure must be current and unrestricted.
• Minimum 1 year of clinical nursing experience.
• CPR certification required.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient/family's physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, appropriate goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and other members of the IDG in providing daily care.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.

About This Role
