Job summary

Carilion Clinic in Rocky Mount, VA is seeking an RN Case Manager for Hospice to provide individualized nursing care to patients across age groups in home settings. Responsibilities include assessment, planning, and coordination of care, collaboration with the hospice interdisciplinary team, and documentation in EMR systems.

A valid Virginia RN license, ability to drive, and strong critical thinking are required.

Qualifications

• Graduate of accredited Registered Nurse Program.
• Virginia RN license or compact state license.
• Legally eligible for home visit driving may be required.
• Strong critical thinking and prioritization skills.

Responsibilities

• Provide nursing care and education to patients across ages in hospice.
• Assess, plan, implement, and evaluate patient care with the interdisciplinary team.
• Document all care accurately in the EMR and ensure orders are current.
• Coordinate care with family/c caregivers and providers to prevent readmissions.
• Administer medications and treatments per care plans.
• Support patient safety and comfort during transitions of care.
