Job summary

Carilion Clinic seeks a Hospice RN in Rocky Mount, VA to provide compassionate end-of-life nursing care and coordinate symptom management across care settings. The role involves admissions, care planning, and collaboration with the interdisciplinary team to support patients and families.

Qualifications

• Graduate of an accredited registered nurse program.
• Currently licensed as a Registered Nurse in Virginia or approved compact state.
• Valid driver's license, automobile insurance, and personal vehicle for home visits.
• Life Support: BLS-HCP certification.

Responsibilities

• Provide end-of-life nursing care and symptom management.
• Coordinate care with interdisciplinary team and admissions.
• Perform comprehensive assessments and develop individualized plans of care.
• Administer treatments and medications and document care in EMR.
• Support patient and family rights, psychosocial and cultural needs.
• Participate in team meetings and ensure regulatory compliance and documentation.
