Serenity Hospice PA is seeking an experienced Hospice RN Case Manager for a dedicated facility-based role in West Chester, Pennsylvania. You will manage individualized care plans for patients residing in the facility, collaborate with facility staff, and provide compassionate, expert clinical leadership.

This position offers a Monday-Friday schedule (8:30 a.m. to 4:30 p.m.) with no nights or weekends, and a strong interdisciplinary team to support patient-centered care.

Formación

• Experience in hospice care and case management.
• Excellent communication with patients, families, and staff.
• Ability to lead and collaborate with interdisciplinary teams.

Responsabilidades

• Manage and coordinate individualized hospice plans of care for patients in the facility.
• Perform comprehensive nursing assessments and routine visits.
• Monitor patient condition and provide timely symptom management.
• Educate patients and families about end-of-life care and hospice philosophy.
• Coordinate medications, equipment, and hospice services.
• Maintain accurate clinical documentation and participate in team meetings.
