Job summary

UnitedHealth Group’s Heart of Hospice Lancaster is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients. You will collaborate with clinicians to implement evidence-based treatment strategies recognizing physical, psychosocial, and spiritual well-being.

Key duties include initial eligibility evaluation, ongoing needs assessment, care coordination, and family instruction on self-care. A valid RN license and CPR are required, with a clean driving record.

Qualifications

• Current and unrestricted RN licensure in the state of practice (SC).
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• 1+ years of clinical experience (preferred).
• Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family and physicians to provide daily care.
• Instructs patient/family in self-care techniques when appropriate.
