Job summary

Amedisys, part of the Optum family, is seeking an RN Case Manager to coordinate compassionate hospice care in the Boston area. You will assess patients, collaborate with the interdisciplinary care team, and develop plans to meet each patient’s needs.

This role offers a $10,000 sign-on bonus for external candidates, a salary range of $91,540 to $137,310 annually, and a comprehensive benefits package. Join us to deliver personalized care in the home setting and grow with a supportive team.

Qualifications

• Current RN license required.
• Valid driver's license and reliable transportation.
• Current CPR certification required.
• 1+ years of nursing experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records according to center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with the clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
