Job summary

AccentCare in San Antonio is seeking a Hospice Registered Nurse (RN) to join our interdisciplinary care team. You will provide compassionate, patient-centered nursing care focused on comfort and quality of life for hospice patients and their families.

You will collaborate with physicians, families, and teammates to develop and carry out personalized care plans, conduct comprehensive assessments, educate patients and caregivers, and coordinate care across disciplines.

Qualifications

RN license in the state of practice.
Minimum of 1 year nursing experience.
Reliable transportation and valid driver's license.

Responsibilities

Provide individualized nursing care that maximizes comfort and quality of life.
Partner with patients, families, physicians, and the inter-disciplinary group to create and carry out care plans.
Perform comprehensive assessments-physical, emotional, social, and spiritual-to guide care decisions.
Educate and support patients and caregivers, meeting them where they are.
Coordinate care across disciplines to ensure continuity and dignity.
Document care clearly reflecting patient needs and progress; participate in on-call or admission rotations.
Engage in ongoing learning and professional development.
