Job summary

Adoration Health is seeking an experienced RN Case Manager to join our Hospice team in Independence, OH. You will provide patient-centered nursing care, develop care plans, manage symptoms, and educate families in a supportive home-based setting.

This role offers a weekdays schedule, on-call rotation, and opportunities for professional growth within a growing company. Nursing licensure in the state and hospice experience are preferred, with autonomy and collaborative team support emphasized.

Qualifications

• Graduate of an accredited nursing school with current state licensure.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
