Job summary

Phoenix Home Care & Hospice is seeking a Home Health Case Manager in Springfield, MO to coordinate care in patients\' homes and collaborate with a multi-disciplinary team. You will ensure orders are carried out, develop individualized care plans, and perform thorough initial assessments to support optimal outcomes.

The role emphasizes strong clinical assessment, communication with physicians, and coordination across nursing, therapy, and ancillary services.

Qualifications

• Missouri or Compact RN license required.
• Driver's license and vehicle with current auto insurance.
• Home health or critical care experience preferred.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Handle varied weekly caseload with routine visits, evaluations, and starts of care.
• Educate patients on health conditions (CHF, COPD, diabetes, wound care, meds).
• Communicate with physicians to obtain orders and write diagnosis-driven plans of care.
• Collaborate with PT/OT/Speech therapy to support patient goals.
