Job summary

Trinity Health at Home Hospice is seeking a Hospice RN Case Manager to provide professional nursing care in patients' homes in Oakland County. You will assess, plan, and implement the hospice care plan, acting as the case manager for each patient and coordinating with physicians and the broader hospice team.

The role requires an active RN license in the state, at least one year in acute care, and strong communication skills with a focus on grief/loss support.

Qualifications

• Current Registered Nurse licensure in the state of practice.
• Minimum of 1 year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Strong communication and interpersonal skills, with emphasis on grief/loss management.
• Must have a current Driver’s license and reliable transportation to and from work site.
• Commitment to mission and Ethics program compliance.

Responsibilities

• Assess and plan hospice patient care in the home setting.
• Provide one-to-one nursing care and supervise care outcomes as case manager.
• Collaborate with physician and hospice team to manage symptoms and bereavement needs.
