Absolute Hospice is seeking an experienced RN to join our hospice team as a Hospice RN Case Manager in Massillon, OH. The RN will provide direct patient care to hospice patients and support families while coordinating care with a multidisciplinary team.

The role emphasizes assessment, care planning, patient education, and documentation in alignment with the plan of care. A $10,000 sign-on bonus is offered and a variety of healthcare plans are available.

Qualifications

• Licensed RN with hospice or palliative care experience.
• Strong patient-family education and communication skills.

Responsibilities

• Provide direct patient care to hospice patients as part of an interdisciplinary team.

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team!

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group and support staff to enhance the quality of hospice are, and, case manages all assigned patient clinical records. Some duties include, but not limited to:

• Assures the highest quality of care is provided and documented in accordance with the Plan of Care
• Performs the initial and on-going assessment of the patient
• Helps develop and implement the hospice plan of care as prescribed by the physician and in conjunction with the other members of the group
• Responsible for the education of the patient, family or other caregivers including medication administration, dressing changes, oxygen administration, hospice philosophy and care of the terminally ill patient
