Absolute Hospice is seeking an experienced RN to join our hospice team in Green, OH. The RN Case Manager will provide direct patient care and coordinate interdisciplinary services.

The role emphasizes assessment, plan of care development, patient and family education, and documentation to ensure quality end-of-life care. Full-time schedule with a $2,500 sign-on bonus.

Qualifications

• Valid RN license in Ohio.
• Experience in hospice care or end-of-life services preferred.
• Ability to educate patients and families on care plans.

Responsibilities

• Perform initial and ongoing patient assessments.
• Develop and implement hospice plans of care with physicians and team.
• Educate patients and families on medications, dressing changes, and comfort measures.
• Document all clinical records within the care plan.

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team!

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group and support staff to enhance the quality of hospice are, and, case manages all assigned patient clinical records. Some duties include, but not limited to:

• Assures the highest quality of care is provided and documented in accordance with the Plan of Care
• Performs the initial and on-going assessment of the patient
• Helps develop and implement the hospice plan of care as prescribed by the physician and in conjunction with the other members of the group
• Responsible for the education of the patient, family or other caregivers including medication administration, dressing changes, oxygen administration, hospice philosophy and care of the terminally ill patient
