Job summary

Absolute Hospice is seeking an experienced Registered Nurse (RN) to join our hospice team as a Case Manager. The RN will provide direct patient care to hospice patients and support the patient’s family while coordinating an interdisciplinary team to implement and manage the plan of care.

Key duties include initial and ongoing patient assessment, education for patients and caregivers, and documentation per the Plan of Care. A $10,000 sign-on bonus is offered with full-time schedules.

Qualifications

• Active RN license in the state.
• Experience in hospice care and patient education.
• Ability to coordinate interdisciplinary care teams.

Responsibilities

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team!

Full-Time Schedules

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group and support staff to enhance the quality of hospice are, and, case manages all assigned patient clinical records. Some duties include, but not limited to:

• Assures the highest quality of care is provided and documented in accordance with the Plan of Care
• Performs the initial and on-going assessment of the patient
• Helps develop and implement the hospice plan of care as prescribed by the physician and in conjunction with the other members of the group
• Responsible for the education of the patient, family or other caregivers including medication administration, dressing changes, oxygen administration, hospice philosophy and care of the terminally ill patient
