Job summary

Absolute Hospice is seeking an experienced RN to join our hospice team as a Hospice RN Case Manager in Massillon, OH. The RN will provide direct patient care to hospice patients and support families while coordinating care with a multidisciplinary team.

The role emphasizes assessment, care planning, patient education, and documentation in alignment with the plan of care. A $10,000 sign-on bonus is offered and a variety of healthcare plans are available.

Qualifications

• Licensed RN with hospice or palliative care experience.
• Strong patient-family education and communication skills.
• Ability to coordinate care and support an interdisciplinary team.

Responsibilities

• Provide direct patient care to hospice patients as part of an interdisciplinary team.
• Educate patients and families on medications, care plans, and hospice philosophy.
• Develop and implement the hospice plan of care in collaboration with physicians and the team.
