Job summary
BrightSpring Health Services, through its Adoration Health affiliate, is seeking a Registered Nurse Case Manager to join our hospice team in Lawrenceville, GA. You will provide compassionate, patient-centered care at a critical time, coordinating with families and interdisciplinary colleagues to develop and monitor individualized care plans.
The role emphasizes on-call availability, thorough documentation, and adherence to hospice philosophy.
Qualifications
- Graduate of an accredited nursing program with current GA licensure.
- 1 year nursing experience required, 3+ years preferred.
- Experience in hospice or similar setting preferred.
- Knowledge of hospice philosophy of care.
- Commitment to clinical and documentation excellence.
- Must have a valid driver's license and insured vehicle.
Responsibilities
- Assess/monitor physical, emotional, and psychosocial needs of patients.
- Create hospice care plans aligned with patient goals.
- Provide direct nursing care: administer medications, treatments, interventions.
- Manage pain and symptoms effectively.
- Educate and support patients' families and caregivers.
- Collaborate with interdisciplinary team.
- Maintain accurate and timely documentation.
- Participate in on-call rotation as required by local branch.
