Job summary

BrightSpring Health Services is seeking a dedicated RN Case Manager for Adoration Hospice in Parkersburg, WV. You will provide patient-centered hospice care, assessing needs and creating care plans while coordinating with an interdisciplinary team.

The role requires an active RN license, hospice experience (preferred), and strong documentation skills. Schedule is typically Monday–Friday, 8am–5pm, with on-call rotation as needed.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile and CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients' wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
