Job summary

BrightSpring Health Services is seeking a Registered Nurse Case Manager for hospice services in the Atlanta metro area. The role involves comprehensive patient assessment, care planning, and direct nursing care to support patients and families in their time of need.

You will collaborate with an interdisciplinary team, ensure clear documentation, and participate in on-call rotations. A nursing license and relevant hospice experience are required or preferred.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with automobile and insurance as required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
