Join Serenity Hospice in York County, Pennsylvania as an RN Case Manager to coordinate hospice care for your patients, perform assessments, and develop care plans in collaboration with the hospice team.

Qualifications

• Active Pennsylvania RN license required.
• Hospice or home health experience preferred.
• Excellent communication and organizational skills.
• Compassionate, patient-centered approach.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate and manage hospice care for assigned patients.
• Perform comprehensive patient assessments and develop individualized care plans.
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide compassionate education, guidance, and support to patients and families.
• Maintain accurate and timely documentation in accordance with hospice regulations.
