Serenity Hospice, based in Lancaster, PA, is seeking an experienced Registered Nurse Case Manager to coordinate and manage hospice care for assigned patients. You will perform comprehensive patient assessments and develop individualized care plans, collaborating with physicians and the interdisciplinary team.

Qualifications

• Hospice or home health experience preferred.

Responsibilities
