Job summary

Miracle City Hospice is hiring an RN Case Manager. Join our compassionate team dedicated to helping patients and families facing end-of-life journeys. Comprehensive benefits, including PTO and 401(k) match, are offered.

The role focuses on leading nursing processes, coordinating care, and ensuring high-quality patient outcomes within the Willoughby to Geneva territory. Shift: Mon-Fri 8a-5p; strong emphasis on family support and documentation.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data and assess information from patient factors that affect needs and coordinate with interdisciplinary team.
• Initiate communication with the attending physician and other agencies for coordinating optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process


• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team


• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care


• Provide regular visits to re-assess the patient's needs and update the plan of care


• Perform clinical pronouncement of death and provide support to the family


• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely


• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
