Job summary

VNA Hospice NWI is expanding and seeking compassionate, skilled Registered Nurses to join our interdisciplinary hospice team. As an RN Case Manager, you will coordinate and deliver high-quality hospice care to patients in their homes, ensuring comfort, dignity, and support for both patients and families.

Schedule: Monday–Friday, 8:00 AM–4:30 PM with on-call requirements; Service Area includes Porter, LaPorte, Jasper, Lake, Newton & Starke Counties.

Qualifications

• Current Indiana RN license (required).
• Minimum associate degree in nursing from an accredited program.
• At least one year of medical/surgical, long-term care, ICU, ER, or home health experience preferred.
• Hospice experience strongly preferred or willingness to embrace hospice philosophy.
• Current CPR/BLS certification.
• Valid driver’s license and auto insurance.
• Sensitivity to issues of loss, grief, and end-of-life care.

Responsibilities

• Provide comprehensive nursing assessments and ongoing evaluation of patient needs.
• Develop, implement, and revise individualized Plans of Care.
• Coordinate services with the interdisciplinary team (physicians, social workers, aides, chaplains, etc.).
• Educate patients and families on disease progression, symptom management, and care techniques.
• Perform physical assessments, manage medications, and ensure timely documentation.
• Arrange for necessary DME, supplies, and medications.
• Function independently while managing the demands of a dynamic caseload.
