Job summary

Elara Caring in Texarkana, TX is seeking a Registered Nurse Case Manager to deliver high-quality hospice/home health services in patients' homes. You will assess needs, coordinate care, educate families, and collaborate with physicians and the care team.

Requirements include current RN license, graduation from an approved nursing program, 1+ year of clinical nursing experience, ability to lift up to 50 pounds, and a valid driver’s license or reliable transportation; hospice/home health

Qualifications

• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Physical ability to lift, push, pull, and assist up to 50 lb.
• Valid driver’s license or reliable transportation to travel to patient homes.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual, and environmental domains.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
