Qualifications

• Healthcare experience in homecare, hospice, or related field preferred.
• Ability to complete documentation legibly and timely.
• Scheduling experience preferred.
• Interpersonal, verbal and written communication skills required.

Responsibilities

• Receive referrals by phone or in person for designated department.
• Explain benefits to patients, families and community contacts.
• Assist with finding alternative services when hospice candidacy is not met.
• Oversee scheduling of initial nursing assessment and admission.
• Maintain admission packets and ensure forms are available to admissions staff.
• Assist with admission paperwork and adherence to work schedule.
• Provide telephone and clerical support to staff as time allows.
• Maintain accurate clinical records showing client goals and progress.

Qualifications

• Be eighteen (18) years of age or older
• At least one (1) year experience in homecare, hospice, and/or healthcare-related field preferred
• Ability to complete necessary documentation appropriately, legibly, and timely
• Scheduling experience preferred
• Ability to handle stressful and delicate situations
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
• Valid driver’s license and proof of insurance are required

HOURS

Monday-Friday 8:30AM-5PM Including one summer and one winter holiday/annually.

On-Call Weekend Rotation Expectation

1x/month on-call weekend rotation in-office 8:30AM-5PM (Saturday and Sunday)

LOCATION

Palos Heights, IL.

Training Requirement

This position will be permanently based in our Palos Heights office. However, the initial training will take place at our Deerfield, IL office Monday-Friday for at least the first 4-6 weeks of employment depending on prior experience. Candidates must be able to reliably commute to the Deerfield location throughout the training period before transitioning to the Palos Heights location

Compensation

$20-$28 per Hour - Offer Based on Years of Experience

What You’ll Do

• Receive referrals by phone or in person for designated department
• Provide an explanation of the benefits to patients, families, and community contacts
• Assist with finding alternative services when the patient is not a candidate for hospice
• Oversee scheduling of initial nursing assessment and admission
• Maintain admission packets and ensure that proper forms are available to admissions staff
• Assist community outreach representatives with marketing activities to promote relations with referral sources
• Assist with the completion of admission paperwork as needed
• Adhere to work schedule and maintain good punctuality and attendance record
• Provide telephone and clerical support to other staff members as time allows
• Maintains accurate clinical records, keeping track of goals and progress of clients’
