Job summary

Interim HealthCare Home Health and Hospice in Michigan is seeking a Hospice RN Case Manager to deliver skilled home-based nursing care with autonomy and meaningful one-on-one patient relationships.

You will coordinate with physicians, oversee LPNs and aides, educate families, and manage care plans while enjoying a healthy work-life balance and supportive teamwork. This salaried role offers weekly bonus opportunities and comprehensive benefits.

Qualifications

• RN with active, unrestricted license in Michigan.
• At least 1 year of clinical RN experience (Hospice preferred).
• Current CPR certification.

Responsibilities

• Provide home-based nursing care to patients in their homes.
• Coordinate with physicians and oversee LPNs, CNAs and aides.
• Assess patients, document progress, and report changes to the physician.
• Educate patients on plan of care, medications, therapy, and home safety.
• Coach family members on care plans and safety.
• Ensure goals are met and coordinate discharge from services.

Hospice Nurse Responsibilities:

• Provide home-based care to patients with illnesses, injuries and chronic diseases-often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
• Examine patients, do assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient's plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services

A few must-haves for Hospice Registered Nurses:

• Registered Nurse (RN) with active, unrestricted license in the State of Michigan - BSN not required
• At least one (1) year of clinical experience as a Registered Nurse (preferably in Hospice)
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance
