Job summary

Ascend Hospice in Paramus, NJ seeks an RN Case Manager to coordinate hospice care, conduct initial assessments, and develop care plans. You will collaborate with the healthcare team to ensure continuous, well-coordinated services for patients and families.

This role requires an Associate's Degree in Nursing, a State RN license, and 2 years of nursing experience (hospice preferred); valid driver's license and willingness to travel are required. Competitive salary and benefits offered.

Qualifications

• RN license issued by the state is required.
• 2 years of professional nursing experience; hospice experience preferred.
• Valid driver’s license with reliable transportation; willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Assist the Team with On Call/Case Management/Visits to cover and call outs.
• Complete initial assessments to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs and adjust care plans.
• Collaborate with the healthcare team, patient, and family for coordinated care.
• Counsel patient and family on end-of-life needs.
• Initiate appropriate preventive and palliative nursing procedures.
• Communicate with the physician regarding patient needs and changes.
• Supervise Hospice Aides.

Now Hiring - RN Case Manager

Starting Salary: 90,000/Year

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills.
