Job summary

IHA is seeking a Hospice RN Case Manager in Mason City, IA to provide compassionate, patient-centered care for hospice patients and families. You will assess, plan, coordinate services, educate, and oversee case management to ensure quality end-of-life care.

Full-time position with a $10,000 sign-on bonus. Requires RN license in IA, CPR, and acute care experience; hospice/home health experience preferred. Valid driver's license and reliable transportation are required.

Qualifications

  • Graduate of an accredited nursing program.
  • Current RN license in the state of practice.
  • CPR certification required.
  • Minimum one year of acute care nursing experience required.
  • Hospice or home health experience preferred.
  • Strong communication, assessment, and relationship-building skills.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Manage a hospice patient caseload and serve as primary case manager.
  • Complete comprehensive patient assessments and develop individualized care plans.
  • Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other interdisciplinary team members.
  • Manage pain and symptoms using evidence-based hospice and palliative care practices.
  • Educate patients and families on disease progression, end-of-life care, and available hospice services.
  • Maintain accurate and timely clinical documentation.
  • Participate in interdisciplinary team meetings and care conferences.
  • Supervise Hospice Aides and LPNs, including required supervisory visits and care plan oversight.
  • Support patients and families through grief, loss, and emotional transitions.
