Job summary

Trinity Health At Home in Mason City, IA is seeking a Hospice RN Case Manager to provide compassionate, patient-centered end-of-life care. You will assess patients, plan care, manage symptoms, coordinate services with an interdisciplinary team, and educate families.

The role requires RN licensure, CPR, and at least one year of acute care nursing. Hospice or home health experience is preferred, with a $10,000 sign-on bonus offered.

Qualifications

• Valid driver's license and reliable transportation.

Responsibilities

• Manage a hospice patient caseload as primary case manager.
• Complete comprehensive patient assessments and care plans.
• Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other interdisciplinary team members.
• Manage pain and symptoms using evidence-based hospice and palliative care practices.
• Educate patients and families on disease progression, end-of-life care, and available hospice services.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings and care conferences.
• Supervise Hospice Aides and LPNs, including required supervisory visits and care plan oversight.
• Support patients and families through grief, loss, and emotional transitions.

Position Summary

The Hospice RN Case Manager provides compassionate, patient-centered nursing care to hospice patients and families. This role is responsible for assessment, care planning, symptom management, coordination of services, patient and caregiver education, and overall case management to ensure quality end-of-life care.
