The Hospice RN Case Manager is responsible for coordinating and managing the care of hospice patients and families. This role serves as the primary clinical resource, developing individualized care plans, managing symptoms, coordinating interdisciplinary services, and providing education and emotional support throughout the hospice journey.
The RN Case Manager also provides functional supervision and direction to Hospice Aides and LPNs while ensuring quality patient outcomes and regulatory compliance.
