Job summary

Concho Hearts Hospice in Little Elm, Texas, seeks a compassionate RN to serve as a Hospice Case Manager, managing a local caseload and delivering skilled nursing care in patients' homes and facilities.

You will complete visits, admissions, recertifications, and develop plans of care, collaborating with physicians and the interdisciplinary team while documenting on a company tablet.

Qualifications

• Active Texas RN license.
• Strong assessment and critical thinking skills.
• Excellent communication and teamwork.
• Ability to work independently in the field.
• Reliable transportation and willingness to travel locally.

Responsibilities
