Job summary

Concho Hearts Hospice in Granbury, TX seeks an experienced Hospice RN Case Manager to lead patient-centered care in home and facility settings. Manage a caseload, develop care plans, and collaborate with physicians and the interdisciplinary team to ensure comfort and dignity for patients.

The role requires an active Texas RN license, strong assessment skills, and the ability to travel locally. You will document care using a company-provided tablet and support families with empathy and education.
