Concho Hearts Hospice in Fort Worth, TX is seeking a compassionate Hospice RN Case Manager to join our full-time team. You will manage a local caseload, provide skilled nursing care in patients' homes and facilities, and collaborate with physicians and the interdisciplinary team to update care plans.

Responsibilities

• Manage a caseload of hospice patients in the Fort Worth area.
• Provide skilled nursing care in patients' homes and facilities.
• Complete routine visits, admissions, and recertifications.
• Develop and update individualized plans of care.
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide education and emotional support to patients and families.
• Maintain accurate documentation using a company-provided tablet.
