Job summary

Queen City Hospice is seeking a Registered Nurse for on-site home health care in the Ohio/Central area. You will manage a patient and family caseload, assess needs, coordinate with the interdisciplinary team, and ensure timely updates to the care plan.

Responsibilities include regular patient visits, data collection on physical, psychological, social, and spiritual factors, communication with attending physicians, death pronouncement when needed, and supervising Home Health Aides in line with

Qualifications

  • Graduate from an accredited registered nursing program.
  • Licensed as a registered nurse in the state of practice.
  • Must possess current CPR and First Aid Certification.
  • Must be knowledgeable of quality assessment and assurance procedures.

Responsibilities

  • Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
  • Obtain data and assess information from patient physical, psychological, social, and spiritual factors that may impact needs and coordinate with the interdisciplinary team.
  • Initiate communication with attending physician and interdisciplinary team for optimal care coordination.
  • Provide regular visits to reassess patient needs and update the plan of care.
  • Perform clinical pronouncement of death and provide family support.
  • Maintain accurate patient records, charts, progress notes, and timely care plans.
  • Supervise and document delivery of care by Home Health Aide according to guidelines.

Schedule: Monday- Friday 8a-5p with two holidays on call each year

What You’ll Do:
  • Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
  • Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
  • Provide regular visits to re-assess the patient's needs and update the plan of care
  • Perform clinical pronouncement of death and provide support to the family
  • Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
  • Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
Qualifications:
