Job summary

DayToDay Health is seeking a compassionate Hospice RN Case Manager in Grand Rapids, MI to deliver skilled nursing care and support to patients and families during end-of-life care. You will collaborate with physicians and the interdisciplinary team to coordinate care addressing physical, emotional, and spiritual needs.

Responsibilities include in-home or facility visits, developing care plans, documenting thoroughly, and participating in on-call rotations.

Qualifications

• Graduate of an accredited nursing program (Diploma, Associate, or Baccalaureate).
• Current Registered Nurse license.
• Minimum one year of clinical nursing experience.
• Reliable transportation, valid driver's license, and insurance.
