Job summary

Advanced Health Care in Carson City, NV seeks an experienced Hospice RN Case Manager to coordinate patient care, perform comprehensive assessments, and implement individualized plans in collaboration with an interdisciplinary team.

The role requires RN licensure, 2+ years hospice experience, strong communication, and the ability to work independently with travel to patient homes.

Full-time, Monday–Friday, 8 AM–5 PM, with benefits including mileage and cell phone reimbursement and provided scrubs.

Qualifications

  • Current RN license in the state of practice.
  • Minimum of 2 years of hospice or related healthcare experience.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Valid driver’s license and reliable transportation for home visits.
  • Current CPR certification.
  • Knowledge of regulatory requirements governing hospice care and documentation.
  • Commitment to compassionate, patient-centered care in a hospice setting.
  • Experience with electronic medical record systems and documentation preferred.

Responsibilities

  • Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
  • Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, per care plans and orders.
  • Coordinate and supervise hospice aides and other professionals involved in patient care.
  • Communicate with patients, families, physicians, and health team to update progress and discuss plans.
  • Monitor patients’ well-being, ensuring comfort and dignity during end-of-life care.
  • Collaborate with the interdisciplinary team to align care with patient wishes and goals.
  • Ensure compliance with regulatory guidelines and documentation policies.
  • Provide bereavement support to families as needed.
  • Participate in team meetings to discuss plans and progress.
  • Perform other duties as directed by Hospice Director or supervisor.

Description

The Hospice RN Case Manager is responsible for coordinating and managing the care of patients in a hospice setting, providing skilled nursing services and overseeing the implementation of care plans. This role requires strong clinical knowledge, excellent communication skills, and the ability to manage patient care autonomously while collaborating with an interdisciplinary healthcare team.

Schedule: Full-Time, Weekdays (M-F), 8 AM - 5 PM

Primary Responsibilities and Duties
  • Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
  • Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
  • Coordinate and supervise the activities of hospice aides and other healthcare professionals involved in patient care.
  • Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
  • Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
  • Collaborate with the interdisciplinary team to develop and implement patient care strategies that align with patients' wishes and goals.
  • Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
  • Provide bereavement support and resources to patients' families and caregivers as needed.
  • Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
  • Perform all other duties assigned by the Hospice Director or designated supervisor.

Requirements
Job Qualifications
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 2 years of experience as an RN in a hospice or related healthcare setting is required.

Physical Requirements
  • Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.
  • Capable of prolonged standing, bending, squatting, and walking during home visits.
  • Fine motor skills for documentation and administering treatments.
  • Ability to respond swiftly and efficiently to emergency situations.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
