Job summary

Live Well Homecare seeks a Hospice - Registered Nurse Case Manager to assess patient needs, develop and implement personalized care plans, and coordinate with the healthcare team to ensure compassionate, evidence-informed care in home settings.

The role emphasizes advocating for patients, monitoring changes, and updating plans while maintaining safety and cultural sensitivity. RN licensure requires and a driver’s license is essential.

Qualifications

• Nursing Diploma or Associate's degree, RN licensure, valid driver’s license, less than 2 years of experience (preferred Bachelor's degree and 2-5 years experience).
• Emphasis on cultural competence, critical thinking, and safety awareness.
