Job summary

Seacrest Behavioral Health seeks an RN Case Manager to provide compassionate end-of-life hospice care in patients' homes or community settings. You will assess, plan, implement, and evaluate care needs, supervise hospice aides, and coordinate services to ensure patient dignity and comfort.

The role includes on-call caseload management, reporting to clinical leadership, and collaboration with the triage team to admit eligible patients under physician orders.

Qualifications

• Unrestricted RN license in the practice state.
• Valid driver’s license and auto insurance.
• Minimum of two years RN experience, with hospice/palliative care experience preferred.
• Strong clinical judgment, excellent communication, compassion, and ability to handle sensitive situations.
