Job summary

Dignity Care Hospice – A Division of CommuniCare Health Services in Indianapolis, IN is seeking a Patient Navigator / Navigator Liaison (LPN or RN) to guide patients and families through care planning and options within our hospice network.

The role emphasizes coordinating with interdisciplinary teams, educating families about hospice resources, and ensuring smooth transitions while maintaining compliance and compassionate, patient-centered care.

Qualifications

• LPN or RN with a valid nursing license.
• Minimum of 3 years of clinical nursing experience (home health, hospice, or skilled nursing preferred).
• Strong knowledge of advanced care planning and healthcare regulations.
• Excellent interpersonal and communication skills.
• Ability to work independently and collaboratively with interdisciplinary teams.
• Strong organizational and time-management skills.

Responsibilities

• Conduct comprehensive assessments of patients to evaluate medical, emotional, and social needs.
• Collaborate with interdisciplinary teams (physicians, social workers, therapists, etc.) to develop individualized care plans.
• Assist patients and families with advanced care planning, including goals of care, treatment preferences, and end-of-life decisions.
• Serve as a liaison between patients, families, and providers to facilitate smooth care transitions.
• Educate and support patients and families on available hospice and healthcare resources.
• Monitor patient progress and adjust care recommendations as appropriate.
• Maintain accurate documentation of assessments, care plans, and interactions in compliance with regulations.
• Participate in quality improvement initiatives and help develop best practices in patient navigation.
