Job summary

Advocate Health in Illinois seeks a Nurse Admissions Coordinator for our hospice division. You will lead patient access, evaluate prognosis and obtain informed consent for hospice care while coordinating with the IDT and physicians.

This full-time role requires an RN license, BLS, and at least 2 years of direct patient care, plus a valid drivers license. Travel across Chicago and southern suburbs is required, with evenings and weekend shifts in the schedule.

Qualifications

• Registered Nurse license issued by the state.
• BLS for Healthcare Providers certification.
• Valid driver s license.

Responsibilities

• Responsible for the admission to hospice services.
• Leads the interdisciplinary care team specific to each patient and family on their caseload.
• Obtains history and completes physical assessment.
• Utilizes data to assess and to evaluate appropriateness for hospice services and level of care, collaborates with the attending physician, medical director and hospice interdisciplinary team.
• Meets with patients and families to explain the hospice program of services and to obtain informed consent for hospice care.
• Reviews insurance/Medicare eligibility and benefits and documenting data to support billing efforts.
