Job summary

Interim HealthCare Hospice & Palliative Care in Indiana seeks a skilled Hospice Patient Care Coordinator (RN) to oversee care delivered by staff and ensure patient goals are met. The role combines clinical leadership with process improvement and education across the hospice team.

You will supervise, mentor staff, guide admissions, and collaborate with the Director of Clinical Services to optimize outcomes. RN licensure in Indiana and 2+ years hospice RN experience are required; supervisory

Qualifications

• RN licensure in Indiana (active, good standing)
• Minimum 2 years of hospice RN experience
• Supervisory experience preferred
• Valid driver's license and auto liability insurance required

Responsibilities

• Collaborate with the Director of Clinical Services to develop and implement processes to ensure patient outcomes per physician orders
• Provide ongoing education to hospice employees on regulations, standards, policies, and QAPI programs
• Supervise, evaluate, motivate, and develop professional and paraprofessional staff
• Use clinical decision making to oversee the healthcare team for assigned patient population
• Serve as an expert for hospice admission processes and perform admissions as assigned
