Job summary

Enhabit Home Health & Hospice seeks an experienced nursing leader to coordinate hospice services and ensure care is delivered per standards and regulations. You will review patient information at critical care milestones and foster collaboration among physicians, referrals, patients, and caregivers.

The role requires a licensed nurse with 2+ years of nursing experience, including 1+ year in home health or hospice, and solid regulatory knowledge.

Qualifications

• Graduate of an approved school of professional nursing.
• Licensed in the state of practice or compliant with nurse licensure compact rules.
• Two years of nursing experience.
• At least one year in home health or hospice.
• Knowledge of federal, state, and local home care regulations.
• Basic technology skills and operation of a mobile device.
• Management experience is preferred.

Responsibilities

• Coordinate hospice services per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
