Job summary

Enhabit Home Health & Hospice is seeking a licensed registered nurse to lead and coordinate patient care in a home health setting. The role ensures services align with standards, reviews patient data at key milestones, and acts as a liaison among physicians, referral sources, patients, caregivers, and employees.

The ideal candidate will have 2+ years of nursing experience, at least 1 year in home health or hospice, and strong knowledge of applicable laws and mobile technology.

Qualifications

• Graduate of an approved school of professional nursing.
• Licensed in the state you practice or eligible under nurse licensure compacts.
• At least two years of nursing experience.
• One year in home health or hospice.
• Knowledge of federal/state/local laws governing home care operations.
• Basic technology skills, including use of a mobile device.

Responsibilities

• Coordinate overall delivery of home health services to patients per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
