The ideal candidate is a licensed registered nurse with 2+ years of nursing experience, including 1 year in home health or hospice, and solid knowledge of applicable laws and technology use.

Qualifications

• Graduate of an approved school of professional nursing.
• Must be licensed in the state of practice or per licensure compact rules.
• Two years of nursing experience.
• One year of home health or hospice experience.
• Knowledge of federal, state, and local home care regulations.
• Basic technology skills, including mobile devices.

Responsibilities

• Coordinate hospice services for patients following standards and procedures.
• Review and approve patient information during care start, recertification, or evaluation visits.
• Facilitate relationships among physicians, referral sources, patients, caregivers and staff.
