Job summary

Enhabit Home Health & Hospice is seeking a qualified registered nurse to lead and coordinate home health services, ensuring care meets standards and regulatory requirements. The role focuses on collaboration with physicians, patients, caregivers, and staff to deliver high-quality, patient-centered care.

The ideal candidate holds an RN license, has at least two years of nursing experience with at least one year in home health or hospice, and demonstrates knowledge of applicable laws.

Qualifications

• Graduate of an approved school of professional nursing.
• Must be licensed in the state where practicing (or in the nurse licensure compact).
• Minimum of two years of nursing experience.
• Minimum of one year in home health or hospice.
• Knowledge of federal, state, and local laws governing home care.
• Basic technology skills including mobile devices.

Responsibilities

• Coordinate overall home health services to ensure standards of practice are met.
• Review and approve patient information during start of care, recertification, resumption, or evaluation visits.
• Facilitate collaboration between physicians, referral sources, patients, caregivers and staff.
