Job summary

AccentCare in Edison, NJ is seeking a Hospice RN to join an interdisciplinary team delivering compassionate end-of-life care. This full-time, on-site role emphasizes autonomy with support from a mission-driven organization.

You will provide individualized nursing care, develop care plans with families and physicians, perform comprehensive assessments, educate patients and caregivers, coordinate cross-disciplinary care, and participate in on-call rotations.

Qualifications

• RN license in the state of practice
• At least 1 year of nursing experience
• Reliable transportation and valid driver’s license

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans
• Perform comprehensive assessments—physical, emotional, social, and spiritual—to guide care decisions
• Educate and support patients and caregivers, meeting them where they are
• Coordinate care across disciplines to ensure continuity and dignity
• Document care clearly and accurately to reflect patient needs and progress
• Participate in on‑call or admission rotations as needed
• Engage in ongoing learning and professional development
