Job summary

AccentCare Hospice RN opportunity in Connecticut offers a Monday–Friday, 8am–5pm schedule and a pay range of $90,000–$105,000 based on experience. You will join an interdisciplinary care team delivering compassionate end-of-life care to patients and families across multiple towns in the coverage area and will work with autonomy supported by a mission-driven organization.

Qualifications

• RN license in the state of practice.
• At least one year of nursing experience.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments-physical, emotional, social, and spiritual to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
