Job summary

Graham Healthcare Group is hiring a Registered Nurse (RN) for a Hospice role delivering compassionate end-of-life care in patients' homes in the Middleburg, PA area. Eligible for a total bonus potential of $15,000, including a $10,000 first-year success bonus and a $5,000 sign-on bonus after 30 days.

You’ll work with an interdisciplinary team, travel to patient homes, and impact quality of life while building your career in a nationally recognized home health leader.

Qualifications

Current RN license in the state you work in.
Hospice experience preferred for patient-focused end-of-life care.
Must have a valid driver’s license and be able to travel ~20% of time.

Responsibilities

Deliver patient-centered hospice care in patients' homes with a compassionate, interdisciplinary team.
Conduct comprehensive assessments, symptom and pain management, and care planning.
Educate and support patients and families through the end-of-life journey.
Document thoroughly in EMR and maintain compliance.
Work independently while staying connected to the team and schedule.
Travel to patient homes within assigned territory.
