Job summary

Suncrest Hospice in Dayton, OH seeks a dedicated Registered Nurse to provide in-home hospice care, admissions, and symptom management for a group of patients. You will visit private homes and facilities, work with a multidisciplinary team, and respond to patient and family needs.

The RN will be on call after hours, seven days on and seven days off, and travel across the service area as required. A valid RN license is required, with at least one year of experience preferred.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in the state of practice.
• Admissions and On-call experience preferred.
• Works well within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work 1-1 with patients and with a care team.
• Capable of managing visits, documentation, and meetings; understands hospice rules/regulations.
• Willing to drive with reliable transportation to travel across the service area.

Responsibilities

• Provide care and services for hospice patients in homes, facilities or hospitals.
• Complete admissions and patient visits as needed.
• Answer calls from patients, families, and customers.
• Perform symptom management nursing visits; manage admissions and deaths.
