Job summary

Suncrest Hospice seeks a dedicated Registered Nurse (RN) to provide compassionate hospice care to patients in private homes, assisted living facilities, and hospitals within the service area. The RN will handle admissions, patient visits, and on-call duties, communicating effectively with families and the care team.

Ideal candidates hold an active RN license with at least one year of experience and are comfortable traveling.

Qualifications

• Current RN licensure in good standing in the state of practice.
• Admissions and on-call experience preferred.
• Works well within a team with nurses, social workers, chaplains and CNA leadership.
• Willing to drive with reliable transportation to travel across the service area.
• Strong time management and communication skills are essential.

Responsibilities

• Provide care and services for hospice patients in homes or facilities; visit patients as needed.
• Complete admissions and patient visits; respond to calls from patients and families.
• Perform symptom management visits and on-call duties.
• Collaborate with patient care team to advocate for hospice services and Suncrest Hospice.
