Visiting Nurse Association of Wisconsin, Inc. is seeking an experienced Hospice Registered Nurse to provide after-hours triage, assessments, and interventions for hospice patients in home settings. You will support families, coordinate with physicians, and ensure symptom control and safety.

The role requires a valid RN license, BLS, and the ability to travel to patient homes. Strong communication and critical-thinking skills are essential in this fast-paced, compassionate environment.

Formación

• Completion of an accredited or approved nursing program.
• Active RN license in the state of practice.
• BLS certification – American Heart Association.
• Valid driver’s license required for home visits.

Responsabilidades

• Incorporate responsibility, accountability, critical thinking, and sound clinical decision-making into daily nursing practice.
• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms, provide education, and determine appropriate interventions.
• Conduct in-person visits to manage acute symptom changes, provide crisis support, and ensure patient comfort and safety.
• Manage complex end-of-life symptoms, including pain, dyspnea, agitation, anxiety, nausea, and secretions.
• Initiate and carry out physician orders and standing hospice protocols within scope of practice.
• Provide emotional support and education to patients and caregivers, including discussion of treatment choices.
• Collaborate with physicians, advanced practice providers, and team members to communicate changes in condition.
• Document all encounters and handoffs accurately in the EMR for continuity of care.
• Adhere to hospice policies, regulations, and professional nursing standards, including education and safety initiatives.
