Job summary

Empath Health's Tidewell Hospice in Florida seeks an after-hours RN to visit patients at home, long-term care, hospice house, or hospital, providing clinical, educational and emotional support as directed by the Triage team.

Under physician supervision, you assess patients, relieve symptoms, document care in the EMR, and update plans of care. Florida RN license and CPR are required; hospice/acute-care experience preferred; CHPN encouraged within three years; BSN/MSN preferred.

Qualifications

• Graduate of approved nursing program and current unrestricted Florida RN license.
• MINIMUM one year experience in an acute-care hospital environment providing direct patient care; hospice RN preferred.
• CHPN certification strongly encouraged within three years of employment.

Responsibilities

• Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team.
• Under physician supervision, performs patient assessment, manages discomfort and provides symptom relief using palliative/end-of-life care nursing skills.
• Utilizes nursing interventions and teaching skills to maximize patient comfort and quality of life for patient/family.
• Attends patient deaths and provides bereavement support to patient caregiver/family; performs post mortem care as needed.
• Provides accurate and timely documentation in EMR reflecting plan of care and outcomes of interventions.
• Reviews/updates Plan of Care, physician orders, MAR/Pharmacy profile to meet patient/family needs.
